Call for PCSN Community Award Nominations

On Wednesday, October 16, 2019 at 8:30am, the Putnam Community Service Network (PCSN) will honor local organizations and individuals at the 34th Annual Awards Breakfast at the Putnam County Golf Course in Mahopac, NY.

We ask you to express your appreciation to an individual or organization that has made an outstanding contribution in our Putnam community in the area of community and/or human services by nominating them for a Putnam Community Service Network Award.

Click here for a list of the ten award categories and the nomination criteria. You may make more than one nomination, but please use a separate form for each nomination.

Your nominations make a difference by showing our community the broad array of community programs, projects, and services provided by dedicated professionals, volunteers, organizations and youth in Putnam County; and by giving much deserved recognition for their accomplishments.  All of the award nominees and award recipients will be recognized at the Awards Breakfast in October.

Thank you for your cooperation and assistance.

If you have any questions, please call or email Stefanie at sh379@cornell.edu
